1. The application form must be typed on a computer. All fields must be completed, and the form must be personally signed. 2. Please read carefully before applying the Template for Student Club Organization Briefing. 3. To ensure fairness, incomplete or late submissions will be considered forfeited and will not be accepted. 4. The submitted electronic and aper documents must be named "Ming Chuan University Student Organization Club Application Form." 5.
